Open Position: Full-Time Resident Care Coordinator

Rivera Mansions seeks multiple applicants for full-time Resident Care Coordinator positions at our facility in Portland, Oregon. Job duties: Resident Care Planning and Implementation: Develops person-centered care plans that meets the individual needs of the resident and in accordance with physician recommendations and State regulations. Family Services: Facilitates periodic family meetings for the community; families through family care conferences, emails, and phones calls; Acts as a resource and meets the families social and emotional needs while encouraging family involvement; Day-to-day Leadership and Training: Oversees day-to-day operational functions; Financial Management.
